Senior Case Management

Case management is a method of providing services whereby a professional social worker collaboratively assesses the needs of the client and the client’s family. When appropriate the social worker arranges, coordinates, monitors, evaluates, and advocates for a package of multiple services to meet the specific client’s complex needs.

As a Licensed Clinical Social Worker I will address both the individual client’s biopsychosocial status as well as the state of the social system and provide intervention at both the client (patient and family) and system levels. It will be my goal to build and maintain a therapeutic relationship with you so that I can link you and your family to the needed services, resources, and opportunities. The services that I provide include but are not limited to:

Psychosocial assessment/Planning/Intervention

Financial assessment/Planning/Intervention

Patient and family counseling

Resource/Referral

Scheduling medical appointments

Completing/Submitting applications

Teamwork/Collaboration

Patient/Family Education

Patient/Family Advocacy

Discharge planning

Long term planning

Fees:

$125 per hour for case management services

Cancelation Policy:

If you need to cancel your appointment, please do so at least 48 hours in advance of your scheduled session time. You will be responsible for the full fee and charged for the time reserved when cancellations are received less than 48 hours in advance.

Insurance:

I do not participate in- network with any insurance panels. You will be provided with an invoice after each session. At this time I am only accepting private pay.
